This lengthy trail follows the Mad River through a wild and very scenic area of the Entiat Ranger District. The trail begins at the Pine Flats Campground and travels to Forest Service Rd #5700 at Maverick Saddle. High water spills over the trail is a number of places in the spring. This trail ties into the upper Mad River Trail #1409.1 which has a seasonal closure to bikes and stock. Otherwise, it is open to all users.

Directions: From Entiat, Travel northwest on County Road 371 (Entiat Valley Road) to County Road 119A. Take this, then Forest Service Rd #5700 to Pine Flat Campground.
